Your Visit

If you are a new patient, please complete and submit the online Patient Registration Form prior to your upcoming visit.

In order for us to better serve you, it is very important that you bring the following on your first visit to Moss Creek Orthopedic Urgent Care:

  • Photo identification
  • Your health insurance card and co-pay
  • List of current medications
  • Any and all previous x-ray, MRI, CT, or bone scans
  • Your referring physician and primary care physician’s name, address and phone number
  • A list of your questions and concerns
  • Patients under the age of 18 must be accompanied by a parent or legal guardian

Is A Co-Pay Required For Follow Up Visits?

Your insurance policy determines whether or not you have a co-pay for your follow up visits; however, under most insurance policies a co-pay is due at each visit.

Payments & Fees

Below are our policies and information regarding payments and fees for your visit:

  • Insurance copays collected at time of service
  • Insurance coverage and deductibles verified a check-in
  • Self-Pay Patients: Payment is expected at time of service (Please ask for Uninsured Patient Policy if applicable)

Insurance And Payment

Insurance
A patient’s insurance card and a photo ID must be presented at each visit and co-payments are due at the time of service. We will file claims with the insurance carrier as a service to the patient. It is the patient’s responsibility to provide Moss Creek Orthopedic Urgent Care with their current insurance card and notify us of any insurance changes. Deductibles or non-covered charges will be the patient’s responsibility and will be requrested at the time of service. Patients with questions regarding their insurance coverage should contact their insurance carrier directly.

Medicare
We accept fee assignment of Medicare claims; claims will be submitted directly to Medicare by our office. The portion of the fee not paid by Medicare will either be patient liability or filed to a secondary insurance.

Credit Policy
Efforts are made for collection of insurance deductibles, co payments and any other charges not covered by a a patient’s insurance plan at the time of service. Patients are responsible for payment of their account regardless of insurance coverage, Moss Creek Orthopedic Urgent Care does not accept responsibility for collecting on a disputed claim. Moss Creek Orthopedic Urgent Care does accept Visa, MasterCard, Discover Card, American Express, Cash or check.
